He was told to keep information from the crew, and since he was programmed not to lie, the order (programming) to keep the info hidden was conflicting with his original programming, that's why he was getting false reports of malfunctioning equipment, turns out he was the one actually malfunctioning. If you saw or read 2010: The Year We Made Contact, they explain everything.
